QUESTIONNAIRE ORDERING PROCEDURE

Please request your mortgage questionnaires well in advance of the date they are required.  Questionnaires are completed in house.  We do not use a third-party service.  Minimum turnaround time is 5 business days upon receipt of payment.  An additional rush processing fee can be paid for turnaround time within 2-5 business days upon receipt of payment.

PLEASE NOTE:  Our office does not have a standard questionnaire form, as most lenders require their own form to be completed.  Please provide blank questionnaire form for completion.

If there is any uncertainty whether an association is a condominium versus a PUD (Planned Unit Development), please contact our office to verify BEFORE submitting any payment.

 *ALL FEES MUST BE PAID IN ADVANCE, PRIOR TO COMPLETION OF QUESTIONNAIRE*

To pay by credit card, you must submit your questionnaire request using the above link.

If paying by check or money order, please make payment payable to CONTINENTAL PROPERTY MANAGEMENT and mail to the address listed below.  You can submit your request online by clicking here, or you can print and mail a QUESTIONNAIRE REQUEST FORM along with your payment to our office address:

If you would prefer to drop off a request and/or payment in person at our above office address, you are welcome to do so.  Requests and/or payments can be left with our receptionist during our normal business hours, or we have a secure drop box located next to our office door that can be utilized while our office building is accessible, typically Monday – Friday, 8AM-6PM.

UPON RECEIPT OF PAYMENT, WE WILL PROCESS THE REQUEST AND RETURN DOCUMENTATION BY EMAIL.

 **NO INFORMATION WILL BE RELEASED PRIOR TO RECEIPT OF PAYMENT**
